Welcome to Southern Interventional Pain Center. Max Shokat, D.O., is a fellowship-trained, dual board-certified interventional pain physician. Our team treats back, neck, joint and nerve pain at our offices in Thomasville, Lake City and Tallahassee.
At your first visit, we review your history and imaging, examine you, and talk through your treatment options. Our goal is to reduce your pain and help you get back to the things you enjoy, using options beyond medication and major surgery.
Insurance
We participate with Medicare, Georgia Medicaid and most commercial insurance plans. Call your nearest office if you have questions about coverage.
What to bring
- Photo ID and insurance card
- Referral, if your insurance requires one
- Recent imaging reports (MRI, X-ray, CT) and any related medical records
- A list of your current medications
- Your completed New Patient Packet
